How do you add policies and required yes/no acknowledgements to Event forms, so that when an end user requests an event of that type, they are required to acknowledge the policy with a “yes” or “no” response?
Solution Overview
This can be accomplished by adding a Yes/No custom question to the form, and then using the “Question Extended Description” to directly input, link to, or attach the policy.
All three approaches are shown below.
How to Do It
Step 1:
Add a new card to the form.
Many institutions title this card something like, “Policies and Acknowledgements”.
Step 2: Drag a Yes / No Custom Question to the card.
Step 3: Check the “is required” box in the field’s Question Settings.
Step 4: Input the acknowledgement verbiage into the “Question Title”.
Step 5:
Input Text Directly | Link to a Policy That’s Housed Elsewhere | Upload a File
Click into the “Question Extended Description”.
Here you can either:
Input the Acknowledgement directly (screenshot 1)
Link to a policy that’s housed elsewhere (screenshot 2)
Upload a file that is then presented to end users as a link (screenshot 3)
Input Text Directly
Input text.
Format as needed.
Click “Save”.
Link to a Policy That’s Housed Elsewhere
Input text into the “Yes No Extended Description” modal.
Select the text that you wish to hyperlink elsewhere.
Click the “link” icon.
Input the URL.
Click “Save”.
Upload a File
Click the “link” icon.
“Click to select file” to locate the file on your hard drive.
Input hypertext for the link (optional).
Click “Upload”.
Click “Save”.
Step 6: Save your form before exiting.
